Appointments
To schedule an appointment, please give our office a call at 845-223-3966. Once you make an appointment, we reserve that time for you. If you are for any reason unable to make your scheduled appointment time, please kindly provide us with 48-hours’ notice so we can reschedule.
What to Bring to Your First Appointment
To ensure we can provide you with proper, prompt care, please bring the following items with you for your first visit:
- Insurance Information/Care
- Photo ID
- Any relevant medical records
Please arrive to your first appointment 15 minutes early to fill out paperwork.

If you would like to save time in the office before your first visit, you can download, print, and fill out the forms below and bring them with you to your first appointment.
Insurance information
We accept most dental insurance plans. Please take the time to familiarize yourself with your insurance plan, benefit maximums, deductibles, and covered percentages. Our staff is happy to file any forms and paperwork for your dental benefits. But please keep in mind your insurance policy is an agreement between you and your insurance company.
You are responsible for any costs not covered by your insurance company. We do not provide services based on what the insurance companies will cover, but on what is best for you and your smile.
Payment Options
Payment is due at the time of service, unless other arrangements have been made with our staff prior to your appointment. Payment includes any copayments or deductibles.
